Grounds for granting a special swimming card and gym card
A special swimming card is an annual pass that covers the swimming hall admission fee for each visit. A special group gym card allows the purchase of gym visit passes and monthly cards at a discounted price. The eligibility criteria for these cards differ.

Medical documentation
Medical documentation may include, for example, a hospital discharge summary or a doctor’s certificate showing the diagnosis and, if necessary, the severity of the condition. If you are applying for a special swimming card or for a special group gym card based on a back or lower limb condition, the documentation must show the required disability rating (percentage of disability). You do not need to obtain a new doctor’s certificate specifically for this purpose if you already have one for another purpose. The documentation is only required for viewing—we do not collect or store it.
